Andrew De Four, CPA - Founder and President

Andrew De Four, is the Founder & President of Customized Homecare, a Montreal-based home care Agency. Andrew founded Customized Homecare in 2013 to respond to families’ desire for their loved ones to age at home safely and with dignity, while receiving the specialized care they may need for chronic or critical illnesses.

Andrew worked in industry for over 15 years at senior levels in finance and in procurement. He worked for many national and global companies such as Telus, Air Canada, Abbott Laboratories, Pfizer and General Electric. Andrew served on the Board of Directors of Seniors Action Quebec. Andrew De Four is a Certified Professional Accountant (CPA) and member of l’Ordre Des Comptables Professionels Agréés du Québec. He holds a bachelor’s degree in accounting from Concordia University.



Souk Simmalavong – Director of Operations

In Souk’s current position as the Director of Operations at Customized Homecare, she maintains a holistic view, being dedicated to customers, and her team alike, as she leads by leveraging her prior experiences in operations and business development. Souk’s goal of maintaining excellent customer relations combined with her hands-on approach and strong people skills have resulted in the implementation of several time-saving strategies which have improved customer service and efficiency.

Souk joined the team as she was moved by the level of compassionate and individualized care we provide our families. Her natural upbeat and engaging manner makes her a pleasure to work with daily, for colleagues and clients alike.



Fay Hinkson - Director of Associate and Digital Business Development

Fay is responsible for the development of our network across markets, in person and online, and ensuring our clients feel supported and have the consistent level of care they have grown to expect from Customized Homecare. Fay has over twenty years’ international experience in market research, analytics and digital marketing strategy. At the heart of her work has always been the client: using empathy to understand their needs and translate that into services and products that best serve them.

Passionate about supporting options for seniors’ and their families’ well-being, Fay has been personally conducting research on aging well for many years, which naturally led her to us. She has worked across a number of industries, including pharmaceutical, automotive and financial services, for brands such as Merck, Pfizer, Michelin and BDC. She spends her spare time traveling and enjoying nature with family and friends, as well as volunteering in the community.



Nicole Freeman - Family Health Nurse

Nicole Freeman graduated as a Registered Nurse in 2005 from John Abbott College and has built a comprehensive career in diverse medical settings. Beginning as a medical-surgical nurse, Nicole honed her skills in patient care and surgical recovery. With emergency care experience, she managed acute conditions, further developing expertise in high-pressure environments. Her in-depth knowledge spans cardiac care, geriatrics, gynecology, and neurology, serving patients with a wide range of needs. As a triage nurse, Nicole efficiently prioritized patient care based on urgency, ensuring optimal treatment outcomes. Currently, as a staff nurse with Customized Homecare, she provides personalized in-home nursing services, conducts patient assessments, administers medications, and educates families on disease management. Nicole's commitment to compassionate, comprehensive care enhances patient well-being and supports wellness and safety at home.



Grace Mandanici - Program Scheduler

Grace is known for her outgoing personality and exceptional people skills and she thrives in environments where she can connect with people and create a welcoming atmosphere. With previous experience as a dental secretary and in real estate, she demonstrates a passion for helping others and takes pride in making a positive impact on the lives of the people around her, ensuring they feel cared for and valued.

Outside of her professional life, Grace has a deep love for nature, especially being by the water, where she finds joy and relaxation. Family is incredibly important to her, and she treasures the time spent with loved ones. Whether at work or at home, Grace embodies warmth, compassion, and a commitment to community.



Kim Sagun - Care and Programs Supervisor

Kim has hands-on experience in the home care industry managing both clients and caregivers, ensuring smooth communication and efficient operations. Moreover, as a mom of a six-year-old, she masters the art of balancing both work and family life, staying organized and adaptable, no matter what the day brings.

She is dedicated to providing top-notch administrative support, problem-solving on the fly, and keeping everything running seamlessly. Her love for helping others extends beyond her career — she’s passionate about creating positive experiences and building strong relationships with everyone she works with.

In Kim’s downtime, you can find her with her nose in a good book—always eager to learn something new. Traveling internationally is also a passion and she loves learning about new cultures and from others’ experiences. Whether it’s a weekend getaway or a longer adventure, for Kim, travel is one of the best ways to grow and stay inspired.



Sanja Divjak - Care Coordinator

Sanja Divjak is a dedicated professional with a background in medical administration assistance. She spent seven years at Sturgeon Community Hospital, advancing from Housekeeping Team Lead to Supervisor. In these roles, Sonja supervised housekeeping staff, ensuring the hospital maintained high standards of cleanliness and hygiene.

Her leadership contributed to a safe and comfortable environment for patients and staff. Seeking new challenges, Sanja relocated to Montreal to join Customized Homecare as a Care Coordinator. In this capacity, she assesses clients' needs, develops personalized care plans, and coordinates home care services. Sanja's organizational skills and commitment to quality care ensure clients receive comprehensive support tailored to their individual requirements.



Quinton Reid - Data and Administrative Analyst

In Quinton’s role as Data Analyst and IT Specialist he is a gatekeeper for our data, ensuring that client and caregiver information is accurately entered into our database. He also trains new caregivers on how to use our app and provides ongoing support to resolve any issues they may encounter. This position engages both his creativity and problem-solving skills, as he works to find solutions that cater to each caregiver’s comfort level with the app.

As a proactive individual, Quinton takes pride in completing tasks ahead of time and to the best of his abilities. Outside of work, he enjoys reading and music production, which allows him to unwind and explore his creative side.
